I hereby authorize Heritage Ventures to initiate electronic debit and/or credit entries to the bank account listed above for any amounts due under my current lease agreement. I represent and warrant that I am the authorized signer for the specified bank account and have full authority to grant this authorization. This authorization will remain in effect until I notify Heritage Ventures in writing to terminate it, allowing reasonable time for processing, or until the Bank or Heritage Ventures provides me with at least five (5) business days' written notice of termination. I understand that any cancellation will take effect no earlier than five (5) business days after the last transaction has cleared and all outstanding balances on the account have been resolved. I also acknowledge that I may be charged a fee for any returned transactions due to insufficient funds or other reasons as permitted by law. The information provided will be used solely for payment processing and will remain confidential.
